The Additional information section should contain:
1. Name of church or non-profit organization
2. Complete address of above
3. Number of persons over 14 interested in HeartSaver CPR class
4. Time preferred. Consider these choices: 9 am to noon, 1 pm to 4 pm, 6 pm to 9 pm.
5. Projected Saturday date (to be considered by VPNM staff. Two - four weeks preparation time
    expected. Date not secured until confirmation.)
6. Alternate day of the week / date if evening session is preferred.
